Expecting a little mericle? Congratulations! Though it's not the easiest process to complete- We hope to make this journey a lot easier through the steps listed below. As well as downloadable forms with explanatory steps, for you to follow.

This document needs to be taken to the hospital/ clinic, and completed by the doctor or nurse who will be assisting the applicant during her maternity journey. The applicant should complete her section of the form as well.

The UI - 2.7 should be completed by the employer. Indicating what sort of remuneration the applicant received whilst on maternity leave. This document is a requirement; whether the applicant received any sort of remuneration or not.

It is a requirement that the employer declare dates on which the employee commenced their duties with the company. As well as indicating where there were  breaks in between; such as commencement of maternity or illness. And on which dates the employee resumed work. This document must be filled by the employer, together with the salary schedule.

 The UI - 2.1 is the general application form for maternity applicants. This form needs to be completed & signed by the intended applicant. However, the Department of Labour no longer makes use of these forms, but rather submits it electronically. It's still a good idea to complete this form and submit it with all the other documents.

This form will be completed and submitted by the applicant for payment processing. Together with the baby's birth certificate. However, if the baby is still to be born, its required that the applicable doctor/ nurse compete the bottom part of this document.

The UI-2.8 serves the purpose of confirming banking details of the benefit recipient. It must be accompanied by either a 1 month bank statement or confirmation of banking details; stamped by a authorised bank employee. The UI2.8 must also be completed, stamped and signed by such bank employee. The applicant is required to complete the bottom part, as well as sign the document. Please also ensure that the surname of the applicant is the same as the ID document.
