It must be accompanied by either a 1 month bank statement or confirmation of banking details; stamped by a authorised bank employee. The UI2.8 must also be completed, stamped and signed by such bank employee. The applicant is required to complete the bottom part, as well as sign the document. Please also ensure that the surname of the applicant is the same as the ID document.
